To order VirtuOst for your patient, two items are required:

1. Signed and completed VirtuOst Order Form
   • This form needs to be signed by both the ordering physician and the patient.
   • For patients in traditional Medicare ("Part B"), include also a copy of the patient's insurance card (both sides), and, if needed, the VirtuOst ABN Form signed by the patient.
2. DVD containing the patient’s CT scan
   • Either mail a copy of the scan to O.N. Diagnostics, or request O.N. Diagnostics to retrieve it from the radiology facility.

Currently, O.N. Diagnostics accepts Medicare Part B (traditional Medicare) insurance and self-payment. Learn more about reimbursement for BCT.
